Sick Notes & Med3 Certificates

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

After 7 days

You will need a Med3 certificate from your doctor. Please arrange an appointment by contacting reception on 01745 814422.

Extension Notes

If you require an extension of a current Med3 certificate, please contact reception or alternatively, use our eConsult service, providing the required date range covered and any changes to your condition.

Private Sick Notes

Unfortunately, some employers may insist on a sick note from a doctor, even if you have been off work for less than 1 week. Providing a Med3 certificate under these circumstances is not a legal requirement and is therefore not provided for by the NHS. There will usually be a private fee for issuing a sick note under these circumstances.
